About Divine Hope Counseling

Divine Hope Counseling LLC was incorporated in February 2008.  We are a substance use disorder (SUD), nonresidential dual diagnose facility.   We operate in Willmar, MN.  We provide services to Spanish-speaking clients, and we are culturally sensitive.  We are open to providing our services for other language barriers.  We accommodate physical and mental health barriers and adapt services to meet their needs.   Our area of expertise is focused on the needs of women, men, and adolescents with chemical dependency and mental health issues with chemical dependency, prescription drugs, and mental health disorders. 

Specialized programs of care for clients will be assessed according to individual needs, which may include cultural sensitivity such as Native American and African American, a translator for Spanish-speaking only individuals, etc., and requests for other types of communication, older adult education, and specialized senior citizen-oriented programs.  Individualized care and structure also include for all participants, exclusive woman’s program, exclusive men’s program, exclusive adolescent program, family therapy, community integration education, and other individualized special program needs that relate to individual needs in their progression of recovery.   All clients at the time of intake and service initiation are residing in the community, and are able to manage their affairs. Clients’ primary vulnerability is SUD and abuse; when sober they are capable of self-management. Divine Hope Counseling staff receives annual training in CPR and First Aide, HIV, Hepatitis, protecting client rights via confidentiality, Bill of Rights, Vulnerable Adults and Maltreatment of Minors, Infection Control, and Opioid Treatment services in Mn.   At any time, and it comes to the attention of Divine Hope Counseling trained staff in meeting and identifying individual needs, that an individual client’s vulnerability has changed regarding previous abuse that is relevant to minimizing the risk of abuse for clients may be made with the permission of the client.

Divine Hope Counseling Location

Divine Hope Counseling is in the older office building near the Rice Memorial Hospital Minnesota and has a main floor entrance. This location is a mixed neighborhood with residential, homes and apartments, churches, and other offices in near proximity. There is parking available in community parking lots on 2nd Street and 4th Street, on-street parking, and handicap accessibility on the north side of the building.   

 

Safety and Confidentiality

Our location has several offices with separate entries, thus protecting clients’ confidentiality and access. Individuals coming and leaving treatment would most often be in groups and would have the ability to request an escort from staff if there were concerns.  Mixed genders amongst clients do not occur unsupervised within the building during therapy hours/sessions by Divine Hope Counseling Staff. 

If clientele should be a registered sex offender, adolescents are not active clients at the same time and precautionary measures are addressed within the vulnerable adult plan. 

There is access to two bathrooms in the main group room and in the main hall. 

Doors to offices are closed when staff are not present. 

Divine Hope Counseling does not encourage or permit abuse. 

At this time of day, programming is anticipated, and the environment provides no living arrangements.  Internal programs involve group participation, which will match individual needs according to their personal progression in recovery, education, and one-on-one counseling sessions. 

Group sessions are 2-4 hours long, and clients will come in at about the same time. The adolescent group is co-ed and is restricted to the group room with no access to the street except upon entrance and exit of the group session.  Additional education and community integration assistance will occur on individual needs.  Groups may contain both males and females who are adults during family therapy or lectures; with the option of exclusive gender group upon individual need. 

When evening programs are necessary to meet client needs, every effort will be made to ensure patient safety during leaving, including accompaniment to their vehicle or the public street. 

 

Divine Hope Counseling is pet Friendly

Chosen pets may be in the building, provided that the following conditions are met.  1. Before bringing a pet to work, proof of the pet being current on their vaccinations and the pet being in good health are to be on file.  2. All pets must be well-behaved and under the control of their owner.  Liability insurance will be the responsibility of the owner.  3. If an employee or client is allergic to an animal, precautions will be taken to keep the pet separate from the person who is allergic.   4. Owners of pets are responsible for cleaning up after their pets.  During all hours of operation, a trained staff will be available to clients. 

 

Assessment Prior to Starting a Program

A comprehensive intake assessment and summary are completed during the initial session, providing information important in identifying abuse risk.  When the assessment indicates that the vulnerable adult does not need specific risk reduction measures in addition to those identified in the program abuse prevention plan, the individual abuse prevention plan shall document this determination.  This includes age, gender, SUD history, symptoms of use, mental health history, treatment history, emotional/behavioral assessment, current stressors, employment, and educational and financial assessment. As well as health screening, spiritual, therapeutic/recreation assessment. Also, there is an assessment of the current family and family history and a family of origin section. The assessment ends with a client's abuse history.   During the BioPsychoSocial aspect of the assessment, the counselor will have the ability to grossly assess mental functioning and fragility.  There is a mental health professional on staff and available to consult with concerning vulnerabilities created by mental health issues.

Attention will also be given to treatment placement so as to be placed in the best environment conducive to his/her recovery.  Suppose a counselor has possession of any specific information or suspects a client may be being abused. In that case, that counselor will contact the common entry point at MAARC and/or Kandiyohi County Social Services Vulnerable Adult Specialist for children. This will only be done if the client has given permission. Staff employed by Divine Hope Counseling will be required to have ongoing training yearly to meet the needs of the client and recognize issues that create vulnerabilities within the program and for individual clients.

The client shall participate in the development of the individual abuse prevention plan to the full extent of the person’s ability.  If applicable, the person’s legal representative shall be allowed to participate with or for the person in the plan's development.  The plan shall contain an individualized assessment of (1) the person's susceptibility to abuse by other individuals, including other vulnerable adults; (2) the person's risk of abusing other vulnerable adults; and (3) statements of the specific measures to be taken to minimize the risk of abuse to that person and other vulnerable adults. For the purposes of this paragraph, the term "abuse" includes self-abuse.

Many times during treatment, an individual will reveal circumstances that make them vulnerable to abuse, which were not evident or present at intake. The plan shall be revised to reflect the results of this review, and reports will be filed to a common entry point as required for adolescents or adults. 

The Integrated Team of Divine Hope Counseling reviews the plan at least annually using the assessment factors in the plan.
