Services
Chemical health and mental health support are provided with individual and group sessions that focus attention on the unique relationship between substance use and emotional health.
Our program for gender-specific adults is designed to:
- meet personal needs.
- offer programs during daytime hours.
- maintain therapeutic group sizes of ten or no more than sixteen participants.
- Meet the needs of clients at the personal progression level by providing primary or post-primary treatment care.
- help clients deal with everyday life without using mood-altering chemicals to cope
- individual help in addressing needs in their life, such as jobs, planning out interactions with other professionals, interviewing skills, assistance in appropriate attire and communication skills, assistance in pursuing educational needs
- family sessions are provided with specific educational needs concerning the effects substance use disorder has on family, children, employment, and friends
- we have an integrated team of LADC counselors, Mental Health Professions (MHP), and Care Coordination for the outpatient program.
- To work collaboratively with county, probation, courts, hospitals, outreach mental health programs
Our program for older adults who can participate through commercial insurance or private pay, we are not eligible for MA, includes:
- offer help to men and women
- Provide education on prescription drug abuse
- we are able to provide counseling for substance use disorder and education in their home; women may participate at the main facility if they are able or willing
- group participation may occur in a nursing home facility or senior housing setting if one or more participants are living within the same facility
- family sessions are encouraged to help the family understand prescription drug abuse and how it relates to substance use disorder

- Individualized Treatment Plan to personal needs based on comprehensive SUD assessment, personal requests, and collateral interests
- Holistic approach, incorporating all six dimensions and assisting a person with an interdisciplinary approach and progressive help in developing a strong and supportive recovery network
- Meeting the needs of the person by accommodating the program to meet their needs. For example, adjusting required hours in a week, transitioning to the next phase upon their need and not a fixed number of hours, toleration relapse, and accommodating personal needs in attendance restrictions…
- Professional team meetings which include the person’s legal professional and personal support network
- Collaborative contact with persons' professional and personal network
- Cultural sensitivity, interpreter for non-English speaking persons; presently Spanish, willing to accommodate other languages and cultures.
- Sensitive to mental health concerns or the recognition of a potential mental health concern or needed pursuit of a mental health diagnosis
- Accommodate treatment barriers such as cognitive limitations or restrictions, comprehensive limitations or restrictions, and/or physical limitations or restrictions.
Substance Use Disorder (SUD) Non-residential Treatment
Treatment focuses on the education of substance use disorder, recognizing high-risk situations, developing coping skills and high-risk and recovery plans, and encouraging community support, which includes sober support meetings and a sponsor and individualized assignments in accordance with their comprehensive assessment.
SUD Non-residential Treatment Options
The comprehensive assessment recommendation determines participation with a treatment option. Treatment options that incorporate less than two months include substance use only. Co-occurring treatment is included with intensive outpatient expectations, usually involving three to six months of treatment.
Relapse Prevention Treatment includes 40 to 70 hours incorporating DMV requirements and times of struggling with a relapse or triggers occurring with intervention to prevent a relapse. This includes substance use disorder (SUD) and co-occurring if deemed necessary.
SUD Intervention Treatment Incorporates 70 to 100 hours in fulfilling recommendations of a comprehensive assessment for nonresidential treatment. This includes substance use disorder (SUD) and co-occurring treatment if deemed necessary.
Aftercare treatment involves intensive outpatient care with 170 hours and includes co-occurring treatment if necessary. Treatment occurs upon recommendation of the comprehensive assessment or referent.
Spanish-speaking SUD sessions incorporate SUD assessments, Treatment Plans, assignments provided in Spanish and English, and individual and group sessions. These services are received with an interpreter or with the aid of an interpretation application. Services are provided as outlined and receive only substance use disorder treatment services.
Adolescent SUD Non-residential Treatment
An exclusive adolescent program, ages 13 and up, with co-ed, females and males, participate in intensive outpatient hour expectations with treatment plans that incorporate personal needs. We offer primary and continuing care programs at Prairie Lakes Youth Program (PYLP). The primary care program meets four days a week for three hours a day for a total of 170 hours. We also provide individual counseling with a Licensed Alcohol and Drug Counselor. In addition, family groups will be offered. We offer continuing care programming for individuals transitioning to the Prairie Lakes Youth Non-Secure Program or the Willmar Area. The continuing care program meets two days a week for three hours. We are willing to accept clients who have been discharged from other programs due to behavioral issues. Divine Hope Counseling provides addiction education to all clients. The education includes individual therapy, group therapy, lectures, activities, and visual aids. A Client receives an individual treatment plan that focuses on the needs of this client to help maintain sobriety during and after treatment. The individual meets with the Licensed Alcohol and Drug Counselor, LADC, regularly for therapy and review of their progress in treatment. The individual receives a discharge summary upon completion or termination of treatment. Divine Hope Counseling also provides on-site education as appropriate.

Treatment occurs upon recommendation of the comprehensive assessment or referent. Aftercare Treatment involves both Primary Intensive Outpatient and Continuing Care expectations with 150 with SUD only. 170 hours with Co-Occurring in order to accommodate additional mental health group hours and additional individual sessions if deemed necessary.

Age 55 and better
Services are the same as listed above, with individual needs met.
FAMILY Sessions

Hours accommodated to individuals as need or required with:
- court orders
- substance use disorder/Comphresive Substance Use Disorder Assessment recommendations
- recommendations based on employment needs
- Relapse Prevention or Relapse Treatment alternatives with individualized hours
Mental Health Therapy provided for diagnostic, personally assessed mental health needs, outside of chemical health needs.
Inclement weather includes severe weather conditions such as blizzards and severe thunderstorms. Generally, if schools are closed, the facility is closed. We are usually closed if Willmar is closed due to inclement weather, generally blizzards. If traveling from another area, if schools are closed due to weather conditions in that area, absences are excused. Telecommunication is provided upon notice from the facility.
Client Portal is part of the electronic filing is incorporated through Procentive. A client portal is provided to view and sign personal documents. An email invitation is sent to the client’s email to complete this service.
COVID-19
Divine Hope Counseling values the health and well-being of our clients. We value healing and believe in establishing hope. Significant steps are being taken to continue serving individuals affected by addiction as we are confronted by the spread of the coronavirus disease 2019 (COVID-19). Divine Hope Counseling Infection Control Procedures: Remain 6 feet away from others. Cover coughs. Wash hands frequently and use hand sanitizers. If you feel sick, call in and leave a message, and absence is excused; a doctor’s note is not required. Wipe down chairs with a sanitation wipe. Wipe down the sink area and toilet seat with a sanitation wipe when leaving, and thoroughly wash your hands. Masks are expected to be worn as required. As of 3-25-20 Divine Hope Counseling has provided an authorization for travel letter for clients and staff. We are an essential healthcare facility, and attendance is permitted.
